While treating a patient,J.D.realizes that he knows very little aboutCarla, her personal life and her feelings and asks her about it, but Carla tells him she doesn't usually open up to people because the would think she is crazy. Eventually she opens up to J.D., revealing all her insecurities, including that she thinksTurkandBonnie Chang(Turk's rival surgeon) have something going on together when she watches them taunt each other. J.D. thinks that it is ridiculous and tells Turk about it, who accidentally lets slip to Carla that J.D. betrayed her and told him. Carla gets furious at J.D. and refuses to ever trust him again, as well as telling a hot nurse about J.D.'s secret (that he swaps her shifts to suit him).
Turk brags to Bonnie
Meanwhile,Bonnieis being picked for better and more surgeries thatTurkand is taunting Turk about it. But thenDr. Kelsopicks Turk over Bonnie to come on the "Doctors Without Borders" program with him and Turk gets his own back on Bonnie by taunting her. But his tauntings are cut short whenDr. Wenreveals that he was only picked over Bonnie (even though Dr. Wen recommended her) because she is a girl and Dr. Kelso is a sexist. When Dr. Kelso admits this, Turk allows Bonnie to go on the program instead but she refuses and makes fun of Turk further so he decides to letToddgo on the trip instead because he knows he would spend the whole trip thinking about Bonnie being better than him.
Elsewhere,Elliottries to get noticed by and get more of a relationship withDr. Coxand repetitively tries to have a conversation with him and he eventually allows her to help him: by giving all the patients the bad news on behalf of him, but Elliot soon finds the sadness to overwhelming for her and forgets about Dr. Cox and his new found respect for her, and puts her happiness first.
In adjusting to their new positions in the hospital, the residents embark on a search for different identities.J.D., feeling more like a doctor, proudly dons a white lab coat and attempts to stand up to theJanitorandDr. Cox.Turkfinds himself with intense back pains after being stuck working with Dr. Amato, a very short surgeon. Meanwhile, Elliot is getting some extra attention from Dr. Di Stefano, a handsome surgeon, and Turk's boss, who constantly assigns Turk to work with Dr. Amato to punish him for skipping guy's night out with the other surgeons. When they all get together later,Carlabrands them each with an identity: J.D. is the geek; Turk is the jocky frat boy with back problems; and Elliot is... white, an identity that Elliot hates because J.D. is also white.
A new patient, Mr. Blair, has been admitted for a severe sinus infection and J.D. is treating him. As Mr. Blair's infection persists, J.D. decides to administer a stronger antibiotic intravenously. The patient ends up losing his sense of smell and blames J.D. After much anxiety, J.D. finds that it wasn't his fault after all and confronts Dr. Cox for seemingly placing the blame on him.
Looking to change her straight-laced reputation, Elliot has a one-night stand with Dr. Di Stefano. Soon after, rumors of her tryst spreads like wildfire throughout the hospital. At first she is upset, but later revels in her new reputation as a sex expert.
Carla and Elliot are treating Mrs. Brumbry when Carla realizes that the patient's age disqualifies her from a beneficial G.I. clinical trial. Dr. Cox decides to break the rules and enters her in the trial anyway.Dr. Kelsotakes pleasure in reprimanding Dr. Cox, but later finds that the patient actually was qualified. He apologizes, but Cox states he would have broken the rules anyway.
In an attempt to fit in with the other jocky surgeons, Turk betrays his friendship with Elliot. Later, after having an eye opening talk with Dr. Amato, he stands up against the other surgeons.
