While treating a patient, J.D. realizes that he knows very little about Carla, her personal life and her feelings and asks her about it, but Carla tells him she doesn't usually open up to people because the would think she is crazy. Eventually she opens up to J.D., revealing all her insecurities, including that she thinks Turk and Bonnie Chang (Turk's rival surgeon) have something going on together when she watches them taunt each other. J.D. thinks that it is ridiculous and tells Turk about it, who accidentally lets slip to Carla that J.D. betrayed her and told him. Carla gets furious at J.D. and refuses to ever trust him again, as well as telling a hot nurse about J.D.'s secret (that he swaps her shifts to suit him).

 

Turk brags to Bonnie

 

Meanwhile, Bonnie is being picked for better and more surgeries that Turk and is taunting Turk about it. But then Dr. Kelso picks Turk over Bonnie to come on the "Doctors Without Borders" program with him and Turk gets his own back on Bonnie by taunting her. But his tauntings are cut short when Dr. Wen reveals that he was only picked over Bonnie (even though Dr. Wen recommended her) because she is a girl and Dr. Kelso is a sexist. When Dr. Kelso admits this, Turk allows Bonnie to go on the program instead but she refuses and makes fun of Turk further so he decides to let Todd go on the trip instead because he knows he would spend the whole trip thinking about Bonnie being better than him.

 

Elsewhere, Elliot tries to get noticed by and get more of a relationship with Dr. Cox and repetitively tries to have a conversation with him and he eventually allows her to help him: by giving all the patients the bad news on behalf of him, but Elliot soon finds the sadness to overwhelming for her and forgets about Dr. Cox and his new found respect for her, and puts her happiness first.

When Dr. Kelso's anniversary rolls around, it is the one day of the year that he actually approves equipment that costs money. Turk is responsible for getting an argon laser, and the Janitor tries to get a new mop. The next day, Turk is in a lot of trouble because he was too late in asking Dr. Kelso for the laser, but when he finally decides to go to Kelso, he reveals that his wife is at fat camp, and Turk realizes that he's been lying for years, and blackmails him into getting surgery the argon laser.

 

Meanwhile, the new residents have to hand in case studies to Dr. Kelso, the most important one going to Reno. Dr. Cox tells J.D. that he respects doctors who don't try to exaggerate diseases to try to put in a case study so J.D. decides to stay on Dr. Cox's good side. Meanwhile, the two cannot figure out what's wrong with a board member, Mrs. Warner, who insists on helping Dr. Cox move ahead but Dr. Cox refuses, preferring to move up on his own. When the Janitor makes J.D. stay in line for him to ask Dr. Kelso something, J.D. ends up wandering away, into Mr. Davis' room who decides that J.D. can use him in his case study. J.D. decides to hide this from Dr. Cox, to remain respected. The Janitor uses his old mop to make J.D. fall and spread the dirty water over his face. To stop it from happening again, J.D. buys him a new mop, the Janitor being lost for words.

 

When Mr. Davis retracts his approval on the case study and J.D. is threatened by Dr. Kelso to hand one in, J.D. decides against it to stay friendly with Dr. Cox. But when Dr Cox and J.D. realise that Mrs. Warner has a very unique disease, J.D. is forced to choose between a certain Reno trip or staying with Dr. Cox. He decides to ask Mrs. Warner and with her approval, his case study is accepted and wins as the most interesting. When Dr. Cox lectures J.D. on selling his soul, J.D. tells him that it's okay to ask for help to further his career. While Dr. Cox has less respect for J.D., he realises his point and asks Mrs. Warner for a recommendation.

 

Elliot and Carla bond

 

At the same time, Elliot andCarla realize the only reason they hang out is because of Turk and J.D.. Elliot tries to force their friendship, but Carla doesn't want to be friends unless it comes naturally. After a patient, Mrs. Kellerman, makes them self conscious about their physical appearance, they bond over a glass of wine and talking about how the other is pretty.

Now residents, J.D., Elliot and Turk wish they had more responsibility. Meanwhile, Carla tries to convince Dr. Cox that he still loves Jordan, however he is distracted by Dr. Kelso's order to introduce him at a congratulatory conference, which he quickly refuses.

 

The new residents get their wish when they have a night shift without Dr. Cox or any attendings to fall back on. This scares them but after a pep talk by Carla, they decide to start working. One patient, a police officer injured in a pursuit, in particular shows no signs of improvement or failure so the unanimous decision is to wait and see if he needs surgery. Turk is constantly called to the clinic, with quite a lot of odd cases to go through and J.D. gets to work on other patients. Elliot is sent all the patients by an aloof ER Doctor.

 

When Dr. Cox watches Jordan sleep, he realizes Carla was right and sneaks back into the hospital to talk about this with Carla, while hiding from the residents. Upon getting home, he tells Jordan about his feelings and she tells him that their casual-sex relationship is ruined, and that she doesn't want to watch him destroy himself again over his disdain for people. Dr. Cox convinces Jordan to give him another chance and Jordan agrees on the condition that he gives the speech honoring and introducing Dr. Kelso.

 

After an hour and still no improvement on their patient, Carla lectures them on how they always whine about having no responsability yet get scared when they are left alone. The three opt for surgery and are now confident enough to finish their shifts.

 

When Dr. Cox walks up to the podium, he realizes he can't change and refers to Bob Kelso as the devil, the audience assuming it as a joke but Jordan leaves after seeing no evidence of him changing.

As Turk, Elliot, J.D. and Carla leave the hospital after their first independent night shift, Carla tells another nurse that she switched shifts to look after her friends.
