J.D. can't sleep as his last day as an intern begins. When he gets up, he is reminded of everything that has happened in the past year, Dr. Cox being in love with Carla, Turk knowing about it, J.D. sleeping with Jordan, J.D.'s relationship with Elliot, and all the characters in the show. Arriving at the hospital, Dr. Cox really doesn't care about J.D.'s last day, and is instead preoccupied with a job opportunity by Dr. Kelso, who wants him to stop bothering him about the monetary issues. J.D., Elliot and Turk all try to pawn off Mr. Bober, an annoying patient, on each other but when they realize none of them cared enough to remembered what Mr. Bober looked like, they realize just how insensitive they had become. When Elliot gives Jordan a physical, Jordan deduces that she is still in love with J.D. and gives her hell about it until Elliot snaps at her and says that the whole hospital knows that Dr. Cox and Jordan still have sex, leading Jordan to another realization. Since Mr. Bober doesn't have insurance and does not seriously need surgery, J.D., Turk and Elliot decide to do something about it. J.D. goes to Dr. Cox who agrees to help him and compliments J.D. on the doctor he is becoming. Turk and Carla go to Dr. Wen who agrees to leave a surgery slot open and Elliot goes back to Jordan to try to convince her to back the decision on behalf of the board but she refuses to.

 

When Jordan is at Perry's apartment for a "predictable" booty call, he tells her that he didn't ask for her help with Mr. Bober because she is predictable, and that she needs to stir it up. The next day in the cafeteria, Jordan spills the beans on everybody's secrets: that Dr. Cox is in love with Carla Espinosa and Turk hasn't told her, that the position Dr. Kelso offered Perry was filled, that Elliot still had feelings for J.D., and she finally reveals to Perry that she slept with J.D. the first time they met.

J.D. wakes up the morning after an incredibly awful blowout between all his close friends and co-workers, hoping it was all a bad dream. Turk is sleeping next to him since Carla kicked him out of his own room for not telling her about how Dr. Cox felt about her. At Sacred Heart, J.D. passed Dr. Cox, who is ashamed with Carla, mad at Dr. Kelso for misleading him about a job offer, and furious with J.D. for sleeping with his ex-wife. He also sees Elliot who quickly leaves, ashamed to be seen by J.D. now that he knows she still has feelings for him. After Dr. Cox unleashes his anger on Dr. Kelso, he proceeds to be surprisingly polite to J.D. Meanwhile, Turk is trying to get back on Carla's good side but is confused when he realizes he doesn't know why she is mad at him. Elliot and J.D. have an awkward conversation where J.D. suggests they sleep with each other, giving a reason for Elliot to be mad at him.

 

When J.D. realizes that Dr. Cox isn't yelling at him because he doesn't care about him anymore, he pages everyone to the cafeteria, where everyone argues and nothing is resolved. J.D. is late for rounds and hasn't done any tests or medication for Mr. Zerbo yet. When Dr. Kelso confronts him on this, J.D. is honest and said he did nothing, to which Dr. Kelso compliments him as good doctors know that any patient bounced from ward to ward has a high risk of fever. Shocked at this response, J.D. realizes that doing nothing sometimes works and all people need is time.

 

Eventually, J.D. and Elliot are comfortable to be around each other again, Dr. Cox realizes he just idealizes Carla, Carla lets Turk back into his own room and Dr. Cox starts yelling and caring about J.D. again, putting everything back to normal.

Now residents, J.D., Elliot and Turk wish they had more responsibility. Meanwhile, Carla tries to convince Dr. Cox that he still loves Jordan, however he is distracted by Dr. Kelso's order to introduce him at a congratulatory conference, which he quickly refuses.

 

The new residents get their wish when they have a night shift without Dr. Cox or any attendings to fall back on. This scares them but after a pep talk by Carla, they decide to start working. One patient, a police officer injured in a pursuit, in particular shows no signs of improvement or failure so the unanimous decision is to wait and see if he needs surgery. Turk is constantly called to the clinic, with quite a lot of odd cases to go through and J.D. gets to work on other patients. Elliot is sent all the patients by an aloof ER Doctor.

 

When Dr. Cox watches Jordan sleep, he realizes Carla was right and sneaks back into the hospital to talk about this with Carla, while hiding from the residents. Upon getting home, he tells Jordan about his feelings and she tells him that their casual-sex relationship is ruined, and that she doesn't want to watch him destroy himself again over his disdain for people. Dr. Cox convinces Jordan to give him another chance and Jordan agrees on the condition that he gives the speech honoring and introducing Dr. Kelso.

 

After an hour and still no improvement on their patient, Carla lectures them on how they always whine about having no responsability yet get scared when they are left alone. The three opt for surgery and are now confident enough to finish their shifts.

 

When Dr. Cox walks up to the podium, he realizes he can't change and refers to Bob Kelso as the devil, the audience assuming it as a joke but Jordan leaves after seeing no evidence of him changing.

As Turk, Elliot, J.D. and Carla leave the hospital after their first independent night shift, Carla tells another nurse that she switched shifts to look after her friends.

When Dr. Kelso's anniversary rolls around, it is the one day of the year that he actually approves equipment that costs money. Turk is responsible for getting an argon laser, and the Janitor tries to get a new mop. The next day, Turk is in a lot of trouble because he was too late in asking Dr. Kelso for the laser, but when he finally decides to go to Kelso, he reveals that his wife is at fat camp, and Turk realizes that he's been lying for years, and blackmails him into getting surgery the argon laser.

 

Meanwhile, the new residents have to hand in case studies to Dr. Kelso, the most important one going to Reno. Dr. Cox tells J.D. that he respects doctors who don't try to exaggerate diseases to try to put in a case study so J.D. decides to stay on Dr. Cox's good side. Meanwhile, the two cannot figure out what's wrong with a board member, Mrs. Warner, who insists on helping Dr. Cox move ahead but Dr. Cox refuses, preferring to move up on his own. When the Janitor makes J.D. stay in line for him to ask Dr. Kelso something, J.D. ends up wandering away, into Mr. Davis' room who decides that J.D. can use him in his case study. J.D. decides to hide this from Dr. Cox, to remain respected. The Janitor uses his old mop to make J.D. fall and spread the dirty water over his face. To stop it from happening again, J.D. buys him a new mop, the Janitor being lost for words.

 

When Mr. Davis retracts his approval on the case study and J.D. is threatened by Dr. Kelso to hand one in, J.D. decides against it to stay friendly with Dr. Cox. But when Dr Cox and J.D. realise that Mrs. Warner has a very unique disease, J.D. is forced to choose between a certain Reno trip or staying with Dr. Cox. He decides to ask Mrs. Warner and with her approval, his case study is accepted and wins as the most interesting. When Dr. Cox lectures J.D. on selling his soul, J.D. tells him that it's okay to ask for help to further his career. While Dr. Cox has less respect for J.D., he realises his point and asks Mrs. Warner for a recommendation.

 

Elliot and Carla bond

 

At the same time, Elliot andCarla realize the only reason they hang out is because of Turk and J.D.. Elliot tries to force their friendship, but Carla doesn't want to be friends unless it comes naturally. After a patient, Mrs. Kellerman, makes them self conscious about their physical appearance, they bond over a glass of wine and talking about how the other is pretty.
